Collection notice: The Department of Health and Hospital and Health Service/s (HHS) (referred collectively as ‘Queensland Health’) is collecting your information and that of your child/children, where applicable, in order to organise and facilitate the collation of a photography and video database. Personal information collected by Queensland Health is handled in accordance with the Information Privacy Act 2009. Your personal information and the personal information of your child/children, where applicable, will only be used and disclosed in accordance with the purposes outlined in the Terms and conditions.  It will not be disclosed to other third parties without consent, unless the disclosure is authorised or required by or under law. Your personal information will be saved in an online database for the purpose of storing and retrieving photo/film consent forms.
